PADI just asks for a head and shoulders pic of you and of the specified size- 1 3/4'' by 2 1/4``. The PADI website says that photos from automatic photo booths (the kind you find in malls and amusement parks) are acceptable. You can even submit an electronic copy of your photo for a replacement card.
